We Need To Verify Your Identity
Your privacy is very important to us and we want to verify your identity and secure your explicit consent prior to migrating your prior scans and reports to the new portal. - Your full name
- Your date of birth
- The mobile number registered to the radiology practice
- The name of the clinic & suburb you attended
- Information about the missing scans, such as: date of scan, type of scan, body part, referring doctor
